Medical Information and Consent Form - 2020
Medical Form 1 - Request to Dispense Medicine
This form is used for the one-off or occasional administering of medication, for example, antibiotics.
Medical Form 2 - Administer Medication
This form is used for the administering/dispensing of regular medications at school and Schedule 8 drugs.

Medical Form 3 - Deed of Indemnity
This form is used for the administering/dispensing of Schedule 8 drugs.

Medical Form 4 - Medical Advice to School
This form is used when it is desirable to obtain further information from the prescribing doctor.  Some medical conditions require more complex Health Care Plans including those with asthma, Type 1 diabetes, epilepsy, anaphylaxis or other more complex health procedures. This form is also used for the administering/dispensing of Schedule 8 drugs.

Medical Form 5 - Notification of Change to Medication
This form is used for notifying changes to medication.

Please note that we do not dispense analgesic substances for pain relief.  Aspirin, or medications containing Aspirin, can only be administered with written authorisation by the student's doctor.  Schedule 8 drugs include Codeine, Concerta and Ritalin.

Year 5 Camp Payment

27 Feb 2020

We have a number of outstanding camp fees.  Please note that students will be unable to attend Year 5 Camp on Monday, 2 March 2020 if the camp fee has not been paid, in full, by Friday, 28 February 2020.  If paying by direct credit, please forward a copy of the receipt to office.staquinas@cg.catholic.edu.au as we will be unable to view this credit by Friday.

Assembly News - Friday, 28 February

27 Feb 2020

Assembly on Friday, 28 February will proceed, however Year 5 students will not be presenting at Assembly due to the illness of both teachers.  Year 5's presentation will be rescheduled.  Week 4 Awards will be presented at Assembly this week as planned.

Medical Forms and Medical Plans need to be updated every 12 months, even if you have previously notified the school of medical plans or treatment procedures.  If you have notified the school that your child has a medical condition that teachers and other relevant staff need to be aware of, please note that you will be requested to complete additional relevant forms.  All our medical forms can be viewed here.

Student Residential Address Collection Notice
